Copies of the parent attestation form will be made available to parents that choose to attest to a symptom screening for their child.  These forms will be included in the beginning of the year packet that each child will receive at the drive through or in person orientation.  By signing the form the parent/guardian confirms that the questions on the form have been completed and the health and well-being of the student has been verified daily prior to boarding the bus
